Client Therapist Agreement
Terms and Conditions
Dr Emma Rae Rhead Hypnotherapy
Please read these terms carefully. Booking an appointment with me means agreeing to these terms and conditions detailed below.
1. Therapy is a Joint Effort.
A key factor in the success of therapy, other than who you choose to see, is the collaborative relationship that develops between therapist and client. The services I offer are to be considered a joint effort that we actively work together to achieve your goals. You will be required to be actively engagaed in the process, motivated and prepared to take the actions I recommend to achieve your results.
2. My Commitment to You
All relationships are based on mutual trust and the therapy relationship is no different. I will always do my best to understand you and your issues without making any presumptions. Everyone is different and you are entitled to be you and to hold your views about the world. If I struggle to understand, I will ask.
I will do my best to provide you with a non-judgemental, safe and supportive environment to help you make positive change, and as you are an individual, I will always do my best to adapt to your characteristics and preferences when working with you.
I will always do my best to be myself and to be honest with you so that you will always know where you stand with me.
3. Results of Therapy
Results of therapy vary and are dependent on multiple complex factors, many of which I as the therapist have no control over.
Many clients achieve excellent life-enhancing results with my unique approach, but I can not, and do not, promise you any particular results or quick fixes. It would be unfair to expect this from me. No treatment approach can work for every individual.
By undertaking therapy with me, you accept that I cannot guarantee that you will achieve your desired outcome, despite our best efforts. Fees are independent of results and are in consideration of my time, energy, training and expertise.
4. Therapy is NOT to be considered Medical Treatment
Therapy or any professional service I offer should NOT be considered medical treatment. You maintain full responsibility for seeking and abiding by your own Doctor’s medical advice and treatment. If I feel that you need to seek medical advice, I will recommend that you do so.
5. Session Attendance / Disability Access
All sessions must start and end on time. If you arrive late for a session, I will be unable to extend your appointment time. You will be required to pay for the full session time as booked.
My consulting room is on the first floor. If you have any mobility problems that make this difficult, please do let me know ahead of booking.
I will never disclose any clinical information you share with me to a third party without your explicit consent, unless I am required to do so by law. This would only be if I felt real concern about possible harm to yourself or others or you tell me about anything illegal that I have a duty by law to report.
7. Cancellation Policy
My practice operates a strict 48 hours cancellation policy. If you are unable to attend your appointment, please cancel or reschedule your session with at least 48 hours notice or the full session fee will be payable in all circumstances.
You may be asked for a deposit payment to secure your session. This deposit is non-refundable in all circumstances including cancellation with notice and covers administrative costs.
If you reschedule a session with 48 hours notice, this deposit to be transferred over to the new session,
I reserve the right to cancel any booked appointment at any time, although I will always do my best to give you as much notice as I possibly can. Last minute cancellations from me would only occur in exceptional and unforeseen circumstances, such as personal or family illness. I can accept no liability whatsoever for any financial or other consequences of such cancellation.
8. Fees and Payment
Fees will be clearly agreed with you ahead of booking.
Deposits paid to secure sessions are non-refundable as they cover administration costs.
You may pay for your appointments by bank transfer, online invoices, debit /credit cards (except Amex) or cash at session. Cheques are no longer accepted.
Payments can be made ahead of session by bank transfer.
Fee for a session must be settled in full by the end of that session in all circumstance.
I reserve the right to change fees at any time, but this will not affect fees for any sessions you have already secured with a deposit..
9. The Right to Terminate Therapy
I reserve the right to terminate our therapy agreement at any time and with no explanation. If I am unable to fulfil my obligation to our work, you will receive a full refund for planned work outstanding. If I need to terminate our therapy arrangement because of unacceptable client behaviour, any payments already made will be non-refundable.
You have the right to terminate therapy at any time. Deposits paid on sessions are non-refundable. With 48 hours notice, any payment on sessions booked ahead (other than deposit) made will be refunded in full.
10. Your Responsibility to Book the Sessions you Need
As client, you have full responsibility to book in my diary what you need to achieve your success. I will give you advice on this ahead of booking. As my diary books up weeks ahead and you must make a commitment to you and plan ahead. I will not be holding sessions by in case you might need them.
On this basis, I advice clients with the majority of issues to book a minimum 2-3 hypnotherapy sessions in the diary ahead to avoid large gaps in your treatment. Please check with me if you are unsure.
12. Death or Incapacity of Therapist
If I was to become suddenly incapacitated by serious illness or death during our work together, you agree to be contacted by my current PA to make you aware of this situation and offer you alternative therapist support.
13. Code of Ethics
I adhere to the Code of Ethics of the professional organisations to which I belong including the Complementary and Natural Health Care Council (CNHC) and the General Hypnotherapy Standards Council. I am fully insured to provide hypnotherapy, psychotherapy, life coaching, health coaching, counselling and mindfulness.
14. Complaints Policy
If you are unhappy with any aspect of my service, please do tell me the moment this comes up for you. I will always do my best to resolve the matter for you personally to your satisfaction. If we are unable to resolve this matter to your satisfaction, I will explain to you the complaints procedure.
